If you would like to add to, correct or place a different interpretation upon any of the incidents in the published report please put your comments in writing and e-mail them to safety@bsac.com or post to the following address:
For new incidents please complete a BSAC incident report form and send it to BSAC HQ at the address shown above.
All personal details are treated as confidential.
A full copy of this Published Report can be DOWNLOADED IN PDF FORMAT (560k).

You will need Acrobat Reader to open and compete the form and you can save a copy to your own computer and Email the completed form to safety@bsac.com . This will allow you to complete, save and print the entire document off-line.
